Referring Veterinarian Form

Please fill out referring veterinarian form. One of our veterinarians will call you to talk about your case and give you an estimate of the cost for your client.

Send Medical Records

Please send all of your patient’s medical records, diagnostics, and radiographs to office@stateavevet.com.

Results & Reports

All CTs and abdominal ultrasounds will be sent to be read by a Board Certified Radiologist. A report will usually be available within 24 hrs of the procedure. We will forward that report to the referring veterinarian to discuss with your client.
